Every parent/guardian must read and understand this waiver prior to the child participating in Breakdancing lessons taught by NRG Breakdancing.
The following waiver releases NRG Breakdancing from all claims, all liabilities, all assumption of risks by the parent, guardian, participant, family, executors, legal
representatives as well as the undersigned heirs, next of kin or friends and agrees not to sue the facility, owners, directors, staff, volunteers, business operators, site property owners or leases.
I agree that NRG Breakdancing/Jimmy Chapman has taken steps to reduce the risks and increase the safety of the breakdancing lessons.
I, as the parent/guardian release and forever discharge NRG Breakdancing/Jimmy Chapman for all liability for all personal injuries.
As the parent/guardian of my registered child, I take full responsibility with them participating in breakdancing lessons of his/her own free will.
I am the legal parent/guardian of the participant and I am executing this waiver on behalf of the participant.
